Job summary

CVS Health is seeking a Case Management Coordinator to support the OhioRISE program. This full-time telework role requires home-based work in Ohio with up to 50% travel to member homes and other sites within Western Ohio.

The position focuses on care coordination, education, and collaboration with care teams to improve healthcare outcomes for children and families, involving case management tools and regulatory alignment. Residency in Ohio is required.

Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children's mental health or related fields delivering community-based services to youth.
• 2+ years experience in family systems, case management, or related areas.
• 2+ years experience with Ohio delivery systems and local networks.
• Must reside in the West Region of Ohio; Butler, Hamilton, Clermont, Warren, Montgomery, Allen, Hardin, Defiance, or Lucas counties.
• Willing and able to travel within the assigned region up to 50% of the time; some travel to the New Albany office may be required for trainings/meetings.
• Reliable transportation; mileage reimbursed per policy.
• Willing and able to work beyond core hours as needed.

Responsibilities

• Coordinate and implement care plan activities and monitor progress.
• Evaluate member needs and determine appropriate care approaches.
• Collaborate with care teams to overcome barriers and ensure optimal outcomes.
• Provide coaching and support to empower member self-management.
• Serve as single point of contact for members and address gaps in care.
