Job summary

CVS Health Corporation is seeking a Case Management Coordinator in Ohio to support the OhioRISE program. You will coordinate care, educate members and families, and monitor care plans across child-serving systems while traveling as needed.

Ideal candidates have 2+ years in behavioral health or related fields, strong MS Office skills, and are resident in Ohio. The role requires flexibility beyond standard hours and travel up to half the time.

Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with MS Office Suite applications (Outlook, Word, Excel, SharePoint).
• 2+ years experience in areas like family systems, case management, child development, or related fields.
• 2+ years experience with Ohio delivery systems, including local networks/resources.
• Willing and able to travel within the assigned region up to 50% of the time; Some travel to the New Albany office may be required for trainings/meetings.
• Bachelor's degree or non‑licensed master level clinician.
• Must reside in Ohio.

Responsibilities

• Collaborate and inform the case management process to facilitate healthcare outcomes for members.
• Provide care coordination, support and education using care management tools and resources.
• Coordinate and implement assigned care plan activities and monitor progress.
• Assist in improving medical appropriateness and quality of care through holistic approaches and multidisciplinary reviews.
• Engage with the member and family teams to overcome barriers and ensure appropriate services.
• Document care management activities according to regulatory and company policies.
