Job summary

CVS Health is hiring for a Field-Based Care Coordinator in Ohio. The role requires travel within Northeast Ohio to member homes and locations, about half the time or more, with some trainings at the New Albany office.

The ideal candidate has 2+ years in behavioral health or related fields and a Bachelor’s degree or master’s level in a health-related field. Regular hours are Monday–Friday, with flexibility for after-hours coverage.

Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children's mental health, child welfare, or related fields.
• 2+ years experience in family systems, case management, or child development.
• Willing to travel within the assigned region 50%+ of the time; some travel to New Albany may be required.
• Reliable transportation; mileage reimbursed per policy.
• Ability to work beyond core hours as needed.

Responsibilities

• Coordinate and monitor care plans and member needs.
• Provide care coordination, support and education for members.
• Collaborate with care teams to optimize health outcomes.
• Assist members to engage with their providers in decision-making.
