Job summary

Molina Healthcare in the United States is seeking a Care Manager to support care coordination across the continuum, focusing on improving member outcomes through assessments, individualized care plans, and collaboration with clinicians and the member's support system.

Qualifications

• At least 2 years experience in health care, preferably in care management, or experience in a medical and/or behavioral health setting, or equivalent combination of relevant education and experience.
• Clinical licensure and/or certification required ONLY if required by state contract, regulation, business operating model, or state board licensing mandates.
• Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job related travel requirements, unless otherwise required by law.
• Demonstrated knowledge of community resources.
• Ability to operate proactively and demonstrate detail-oriented work.
• Ability to work within a variety of settings and adjust style as needed - working with diverse populations, various personalities and personal situations.
• Bachelor's degree in a health care related field may be required in some states (dependent upon state/contractual requirements).

Responsibilities

• Complete member assessments within regulated timelines to identify care coordination eligibility.
• Develop and implement care plans with member and team to meet goals.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plans and document interventions and outcomes.
• Maintain ongoing member caseload and outreach.
• Promote integration of services across behavioral health, LTSS, and home/community resources.
• Facilitate ICT meetings and collaboration.
• Educate and motivate member changes using motivational interviewing.
• Identify barriers to care and provide coordination assistance.
• Collaborate with licensed care managers as needed.
• Travel 25-40% depending on state requirements.
