Job summary

CVS Health is seeking a Case Management Coordinator in Ohio to support the OhioRISE program, evaluating member needs, coordinating care plans, and collaborating with multidisciplinary teams to improve health outcomes for children and families.

This full-time field-based role requires travel across Northeast Ohio (50%+), with a preference for candidates in Seneca, Lorain, Medina, Wyandot, or Crawford counties. Strong organizational skills and a commitment to quality care are essential.

Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years in children's mental health, child welfare, or related public sector field.

Responsibilities

• Evaluate referred members' needs/eligibility and plan care accordingly.
• Coordinate and implement care plan activities and monitor progress.
• Collaborate with clinical teams to address quality and access to services.
• Provide coaching and support to empower members in their healthcare decisions.
• Serve as a single point of contact for members and assist with gaps in care.
