Job summary

CVS Health is seeking a Case Management Coordinator for a full-time field-based role in Northeast Ohio. You will travel to member homes and other locations up to 50% of the time, coordinating care and supporting families in Medicaid-related programs.

Ideal candidates have 2+ years in behavioral health or related public services, with strong proficiency in MS Office tools and Ohio delivery systems. A Bachelor’s or equivalent master’s in a health-related field is required.

Qualifications

• Bachelor’s degree or non-licensed master level clinician required.
• 2+ years of behavioral health, social services, or human services experience.
• 2+ years of experience with MS Office Suite applications (Outlook, Word, Excel, SharePoint).
• 2+ years in children's mental health, child welfare, or related public sector health field.

Responsibilities

• Conduct comprehensive evaluations of referred members' needs and eligibility and recommend care plan approaches.
• Coordinate and implement care plan activities; monitor progress and outcomes.
• Collaborate with multidisciplinary teams to improve quality of care and engage members in decision-making.
• Document care management activities in accordance with regulatory and company policies.
