Job summary

CVS Health is seeking a Case Management Coordinator in Ohio to support the OhioRISE program. This full-time field-based telework role requires travel up to 50% to member homes and locations across Northeast Ohio.

The selected candidate will evaluate member needs, coordinate care plans, and collaborate with multiple systems to improve outcomes, while meeting scheduling requirements and maintaining regulatory compliance.

Qualifications

• Bachelor’s degree or non-licensed master level clinician required.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with personal computers, keyboard and MS Office Suite applications (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children's mental health, child welfare, developmental disabilities, juvenile justice, or a public sector human services or behavioral health care field, providing community-based services to children and youth, and their family/caregivers.

Responsibilities

• Be clinically and culturally competent with training and experience necessary to manage complex cases in the community.
• Evaluation of Members and evaluating needs/eligibility and recommending care planning approaches.
• Enhancement of medical appropriateness and quality of care through collaboration and care planning.
• Monitoring, evaluation and documentation of care according to regulatory guidelines.
