Job summary

CVS Health Corporation in Ohio is seeking a Case Management Coordinator to deliver care coordination and education for members through the OhioRISE program. This full-time, field-based telework role requires travel within the Northeast Ohio region up to 50% and a focus on behavioral health and family-centered care.

Applicants reside in Northeast Ohio counties and will partner with MCOs, vendors, and ODM to support specialized services, care planning, and bridging gaps in care, with hours

Qualifications

• Bachelor’s degree or non-licensed master level clinician required.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with MS Office tools (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children’s mental health, child welfare, developmental disabilities, juvenile justice, or related field.
• 2+ years experience in family systems, community systems and resources, case management, child and family counseling/therapy, child protection, or child development.
• 2+ years experience with Ohio delivery systems, including local community networks and resources.
• Willing and able to travel within the assigned region up to 50% of the time; Some travel to the New Albany office may be required for trainings/meetings.
• Reliable transportation required; mileage reimbursed per policy.
• Willing and able to work beyond core business hours, as needed.

Responsibilities

• Be clinically and culturally competent to manage complex cases across child-serving systems.
• Evaluate member needs, eligibility and plan; coordinate care and monitor progress.
• Enhance medical appropriateness and quality of care through collaboration with care teams.
• Monitor, evaluate and document care in line with regulatory guidelines.
• Serve as a single point of contact for members to remediate gaps in care and access.
• Assist members to participate actively with their provider in decision-making.
