The role requires 2+ years in behavioral health or human services and 2+ years of case management, with reliable transportation and mileage reimbursement. Bachelor's or master-level clinician in behavioral health or human services is preferred.

Qualificações

• Bachelor's degree or non-licensed master level clinician in behavioral health or human services.
• Minimum 2 years of experience in behavioral health, social services or human services field.
• Minimum 2 years of case management experience.
• Willing and able to travel up to 50-75% to meet members in Lake County IL and surrounding areas.
• Reliable transportation; mileage reimbursed per policy.

Responsabilidades

• Evaluate referred member's needs/eligibility and propose care plan resolution.
• Identify high-risk factors and coordinate referrals to appropriate care management.
• Coordinate and implement care plan activities and monitor progress.
• Ensure compliance with regulatory and accreditation guidelines.
