The role includes home-based monitoring, crisis intervention, training, and coordination with service providers to promote placement stability. The position requires a Bachelor's in Social Work or related field, at least three years of relevant experience, and the ability to work evenings or weekends as needed.

Qualifikationen

• Excellent organizational, verbal, written, and interpersonal communication skills.
• Demonstrated leadership abilities and the ability to work independently with children and families.
• Knowledge of kinship foster care services, child welfare services, and adoption practices preferred.
• Strong case management and client assessment skills.
• Strong crisis intervention and problem-solving skills.
• Ability to coordinate services and maintain documentation.

Aufgaben

• Complete biweekly home visits to monitor resource homes and treatment goals.
• Provide training and support to resource parents for children's needs.
• Offer crisis prevention strategies and support to families.
• Intervene in crisis situations and support all parties involved.
• Interface with service providers for service delivery and planning meetings.
• Certify and recertify resource homes per requirements.
• Assist with transportation to appointments and activities as needed.
• Maintain documentation and progress notes.
