Job summary

Qualifications

• Bachelor’s degree or higher in social services or related field.
• One year of related counseling or client advocacy experience in mental health, aging, or disability services is preferred.

Responsibilities

• Schedule and conduct face-to-face or telephonic consultation and provide coaching support to clients and caregivers.
• Meet with individuals and caregivers to complete assessments/documentation.
• Develop action plans addressing client needs and barriers using person-centered principles.
• Identify resources and make referrals to meet client needs.
• Maintain records and share outcomes with management or external payors.
• Present program progress to partners and professionals as needed.
