Job summary

Advantage Behavioral Health Systems in Athens, GA is seeking a full-time Care Coordinator to join our CCBHC team. The role blends direct client support with care coordination to improve outcomes for individuals with behavioral health and chronic medical conditions.

You will assist with intake, vital signs, referrals, hospital follow-ups, and community resource linkage, using EPIC for documentation and care management.

Qualifications

• High school diploma or GED required.
• Medical Assistant certification or relevant healthcare experience.
• Minimum of one year of experience in healthcare, behavioral health, public health, or community health.
• Proficiency with Electronic Health Records (EHR) and Microsoft Office applications.
• Bilingual English/Spanish preferred.

Responsibilities

• Room clients and prepare them for provider visits.
• Obtain and document vital signs, height, weight, BMI, and screenings.
• Collect and prepare laboratory specimens per clinic protocol.
• Document client information accurately in the EHR.
• Coordinate care for clients with chronic medical, behavioral health, and substance use disorders.
• Schedule appointments, referrals, tests, and follow-up visits.
• Conduct reminder calls and outreach to reduce missed appointments.
• Track referral completion and follow up on results and treatment.
• Utilize EPIC and other EHR systems to monitor client activity.
• Collaborate with providers, nurses, and care coordinators for continuity of care.
