Wellness Equity Alliance is seeking a Behavioral Health Care Manager to provide field-based, high-touch coordination for Medi-Cal members with complex mental health or SUD needs. You will work alongside street medicine teams across Riverside and San Bernardino counties, connecting primary care, behavioral health, and housing resources to achieve stable housing and continuous care.

Formación

• Master’s degree in social work, counseling, marriage and family therapy, psychology, or a closely related behavioral health field.
• Valid California driver’s license and reliable transportation.
• Direct experience serving people who are unhoused or housing insecure.

Responsabilidades

• Engage members in field outreach at encampments, shelters, and mobile units.
• Coordinate with PCPs, specialists, and housing providers to ensure integrated care.
• Serve as Lead Care Manager and single point of contact among street medicine clinicians, the PCP, county health agencies, MCP, and housing providers.
• Screen for depression and risk using PHQ-2/PHQ-9 and implement safety plans as needed.
• Develop person-centered care plans with housing goals and action steps; update as circumstances change.
• Assist with Medi‑Cal enrollment and navigation for eligible members.
