Job summary

HealthRIGHT 360 is seeking a Care Manager I to connect clients with physical and behavioral health services, housing assistance, and community resources. The role involves field-based work, meeting clients at various locations to coordinate care and support recovery.

You will work within ECM to coordinate care with a multidisciplinary team, document progress, and help clients navigate barriers to engagement. A valid CA driver’s license and First Aid/CPR are required.

Qualifications

• Care management experience.
• Experience working with clients experiencing acute withdrawal from substances.
• Experience working with Medi-Cal, Medi-Care, and private medical insurance plans preferred.
• We will consider for employment qualified applicants with arrest and conviction records.

Responsibilities

• Identify eligible individuals from various locations for enrollment into ECM.
• Engage clients via in-person meetings, mail, email, texts, phone, and outreach.
• Organize client care activities per MCP requirements.
• Share information with the multi-disciplinary care team.
• Implement activities identified in the MCP.
• Assist with intake and required assessments.
• Connect clients to benefits, healthcare, employment, housing, and community resources.
• Monitor progress toward treatment plan goals and provide input.
• Maintain regular contact with providers involved in care.
• Ensure continuous, integrated care among providers.
• Refer and follow up with primary care, mental health, SUD treatment, and community services.
• Collaborate with clients and families for community reintegration.
• Collaborate with outside agencies for community resources.
• Communicate with the behavioral health team.
