Job summary

SAC Health is seeking an ECM Care Manager, Behavioral Health in San Bernardino. The role involves coordinating care for moderate to high-risk patients, linking to community resources, and delivering brief behavioral interventions within an interdisciplinary team.

The position requires an MSW or related field, CA driver’s license with auto insurance, and EPIC certification. Bilingual English/Spanish is preferred, with 1+ year of relevant experience and a team-focused mindset.

Qualifications

• Master's Degree in Social Work required.
• CA Driver's License and Auto Insurance; ACSW preferred; CPR/BLS; EPIC certification required.
• 1+ year of case management and mental health experience in a community setting with homeless/at-risk populations preferred.
• Proficient in MS Office; able to use electronic health records and various databases.
• Fluency in English and Spanish preferred.

Responsibilities

• Provide intensive case management for a set caseload of patients with complex behavioral and medical needs.
• Conduct initial and ongoing behavioral health and psychosocial assessments; monitor for decompensation and crisis.
• Collaborate with interdisciplinary care teams across the continuum.
• Engage new patients and promote programs to patients, families, and staff.
• Meet productivity expectations; prepare and present systematic case reviews.
