Job summary

You will assess behavioral health needs, develop and monitor care plans with members and providers, promote integration of services including LTSS and community resources, and participate in interdisciplinary meetings. Travel of 25–40% may be required.

Qualifications

• At least 2 years health care experience, preferably in behavioral health.
• Licensed Clinical Social Worker or equivalent licensure per state requirements.
• Valid driver's license and reliable transportation for travel.
• Experience with clinical platforms and Microsoft Office.

Responsibilities

• Provides support for care management/care coordination with a multidisciplinary team.
• Develops and implements care plans with member, caregiver, physician and/or other healthcare professionals and support network.
• Monitors care plans to evaluate effectiveness, documents interventions and suggests changes.
• Maintains ongoing member caseload with regular outreach and management.
• Promotes integration of services across behavioral health, LTSS, and community resources.
• Facilitates interdisciplinary care team meetings and ICT collaboration.
• Assesses barriers to care and provides assistance to address concerns.
• Travel 25-40% may be required based on state/contractual requirements.
