Job summary

You will conduct visits, monitor progress, and promote whole-person care while maintaining strong communication with team members.

Qualifications

• 2+ years healthcare experience, preferably behavioral health.
• Active, unrestricted behavioral health license per state requirements.
• Strong written and verbal communication skills.
• Proficiency with data entry and clinical software platforms.

Responsibilities

• Assess behavioral health to determine eligibility for care coordination.
• Develop and implement care plans with member, family, and providers.
• Conduct telephonic, in-person, or home visits as required.
• Monitor and document effectiveness of care plans and progress toward goals.
• Manage ongoing member caseload with regular outreach.
• Promote integration of behavioral health with LTSS and community resources.
• Facilitate interdisciplinary team meetings and collaboration.
• Use motivational interviewing to support behavior change.
• Identify barriers to care and coordinate solutions.
• Provide guidance to peers as needed.
• Travel 25–40% as required by state contracts.
