Qualifications

• LCSW/LMSW/LPCC/LMFT or equivalent license; state-specific requirements.
• Minimum 2 years health care experience, preferably in behavioral health.
• Valid driver's license with reliable transportation for travel.
• Experience with severe mental illness, substance use disorders, or foster care.
• Knowledge of whole person care principles and discharge planning coordination.
• Proficiency with data entry on clinical platforms and standard office software.

Responsabilités

• Completes comprehensive behavioral health assessments and determines care coordination eligibility.
• Develops and implements collaborative care plans with members and healthcare professionals.
• Monitors care plan effectiveness and documents interventions and goal achievement.
• Maintains ongoing member caseload for outreach and management.
• Promotes integration of behavioral health services with LTSS and home/community resources.
• Facilitates interdisciplinary care team meetings and ICT collaboration.
• Assesses barriers to care and assists members to address concerns.
• Travel may be required 25-40% depending on state contracts.
