Job summary

CVS Health is seeking a Case Management Coordinator based in Ohio for field-based telework. You will travel within Western Ohio to member homes and locations as needed, supporting care coordination and education, and working beyond standard hours to meet member needs.

The role emphasizes evaluation, planning, and collaboration with family teams to improve outcomes for children with complex behavioral health needs, while adhering to regulatory and company policies.

Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with MS Office Suite applications (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children's mental health, child welfare, or related public sector field.
• Must reside in the assigned region and be willing to travel up to 50%.

Responsibilities

• Collaborate to inform the case management process and facilitate healthcare outcomes.
• Provide care coordination, support, and education using case management tools.
• Monitor care plan progress and coordinate activities with multidisciplinary teams.
