Qualifications

Responsibilities

• Coordinate and implement assigned care plan activities and monitor progress.
• Evaluate member needs and eligibility and perform care planning.
• Provide coaching and support to empower members in healthcare decisions.
• Serve as single point of contact for members and address gaps in care.
