Job summary

CVS Health in Ohio is seeking a Case Management Coordinator for a full-time field-based telework role. You will travel within Northeast Ohio up to 50% to member homes and locations, coordinating care and supporting families in a behavioral health context.

The role requires 2+ years in relevant fields, a degree in behavioral health, and strong MS Office skills. Expect 40 hours/week, competitive benefits, and a comprehensive pay range.

Qualifications

• 2+ years of experience in behavioral health, social services, or human services
• 2+ years of experience with MS Office Suite applications (Outlook, Word, Excel, SharePoint)
• 2+ years experience in children's mental health, child welfare, developmental disabilities, or related field
• 2+ years experience in case management, family systems, or related areas
• 2+ years experience with Ohio delivery systems and local networks
• Willing and able to travel within the Northeast Ohio region up to 50%
• Reliable transportation and flexible hours including some after-hours work

Responsibilities

• Provide care coordination and education to members and families
• Collaborate with case managers, supervisors, and Medical Directors
• Monitor care plans and evaluate member needs and eligibility
• Escalate quality of care issues and coordinate services
• Serve as single point of contact for members and assist with access to care
