Spring Fertility is seeking a Patient Navigator to join our on-site care team in Danville, CA. You will partner with our Care Team to provide patients with information they need to approach their treatment cycle with confidence, simplifying a complex process and ensuring a seamless transition to the nursing team for cycle management.

This full-time, non-exempt role requires early hours with rotational weekends and holidays.

Qualifications

• Passion for reproductive healthcare.
• Exceptional communication skills.

Responsibilities

• Generate journey maps and calendars based on treatments plans.
• Facilitate, track and document completion of tests and diagnostic procedures.
• Develop trust and positive rapport with all new patients.
• Explain complex medical process and terms in a digestible manner.
• Monitor patient compliance and readiness for treatment.
• Scheduling appointments.
