Job summary

Spring Fertility in Danville, CA is seeking a Patient Navigator to partner with our Care Team and help patients navigate their fertility journey. You will work with nurses to provide information and ensure a seamless transition to the nursing team for cycle management.

Day-to-day duties include creating journey maps, tracking tests, building trust with patients, and explaining medical processes in an understandable way. This is a full-time onsite role with early hours and rotating weekends.

• Exceptional Communication Skills.

Responsibilities

• Generate journey maps and calendars based on treatment plans.
• Facilitate, track and document completion of tests and diagnostic procedures.
• Develop trust and positive rapport with all new patients.
• Explain complex medical processes and terms in a digestible manner.
• Monitor patient compliance and readiness for treatment.
• Scheduling appointments.
