Job summary

UCFS Healthcare Norwich, CT is hiring a Bilingual Care Coordinator to support high-risk families. You will perform intake, assessments, and plan development through home visits and team coordination.

The role requires Spanish fluency, travel, and strong communication. A bachelor’s degree in human services or related experience is required, with benefits and growth opportunities offered.

Qualifications

• Bachelor's degree in the Human Service field with 1 year of social services experience or training
• OR 5 years of relevant Human Services experience
• Experience working with children and families with behavioral health needs
• Home visiting experience preferred

Responsibilities

• Provide outreach and engagement to clients upon referral
• Conduct initial intake and assess eligibility with the family
• Develop care plans using home visits and Ohio scales
• Provide weekly face-to-face services including advocacy and referrals
• Support development of positive parenting and safety planning
• Coordinate Child Specific Team meetings and follow through with recommendations
• Maintain up-to-date case records and weekly reports
