Job summary

Urban Strategies, Inc. is seeking a Family Support Specialist II in New Bern, NC to provide comprehensive case management, referrals, and coordination of community and supportive services for families participating in the Choice Neighborhoods Implementation (CNI) Grant.

You will work with residents to promote economic self-sufficiency, family stability, and access to resources, collaborating with social workers and other staff to help clients achieve goals and address barriers.

Qualifications

• Bachelor's degree in Social Work, Sociology, Psychology, or a related field.
• At least 3 years of experience as a case manager or direct-service role with low-income households.

Responsibilities

• Conduct resident intakes and assessments.
• Complete home, office, and community visits.
• Develop and monitor individualized development plans.
• Manage an assigned caseload.
• Connect residents with community organizations and public agencies.
• Advocate for resident needs.
• Coordinate service delivery.
• Document resident progress in the LEARN system.
• Participate in community and supportive-service meetings.
• Assist with reports, billing documentation, and other program-related duties.
