Qualifikationen

• Lived experience as a caregiver or family member required.
• At least 1 year of experience in behavioral health, human services, family support.
• Excellent written and oral communication; adept with documentation systems.
• Ability to engage and advocate for families; familiarity with caregivers’ needs.

Aufgaben

• Collaborate with care teams to ensure continuity of care for families.
• Provide referrals and concrete resources (housing, food, transportation, access to services).
• Communicate effectively with individuals and caregivers about available services.
• Assist in crisis and safety planning in partnership with youth, families and the care team.
• Coordinate with guardians, foster families, and external agencies to align care plans.
