CommuniCare in Greater New Haven is seeking a Parent Navigator to join our Child & Family Services team. The role provides direct support, advocacy, and guidance to families as they access resources and navigate service systems.

You will build relationships with families, collaborate with specialists, help create plans, and assist with transportation and outreach to extend community supports.

The position is full-time, 37.5 hours per week, with a salary range of $20-$23 per hour, plus benefits.

Qualifications

• High school diploma or equivalent.
• Minimum of three years of experience working with families.
• Knowledge of community resources and social service systems.
• Strong communication, engagement, and documentation skills.
• Ability to work independently and collaboratively as part of a team.
• Strong organizational and time management skills.
• Valid driver's license and reliable transportation.
• Ability to work occasional evenings and weekends.

Responsabilités

• Build supportive relationships with children, families, and caregivers.
• Collaborate with Family Support Specialists, families, and other community supports.
• Assist families in accessing community resources and services throughout the Greater New Haven area.
• Help develop strength-based, family-driven plans of care.
• Advocate for children and families across educational, healthcare, and community service systems.
• Educate, mentor, and empower families to become active participants in their own care plans.
• Transport clients when necessary and perform community-based outreach.
