Job summary

CommuniCare is seeking a full-time Parent Navigator in New Haven, CT, to support families in identifying strengths, accessing resources, and navigating service systems. The role requires collaboration with caregivers and professionals across education, healthcare, and community services, plus outreach in the Greater New Haven area.

Ideal candidates will have a high school diploma, at least three years of family-focused experience, strong communication and organizational skills, and a valid

Qualifications

• High school diploma or equivalent.
• Minimum of three years of experience working with families.
• Knowledge of community resources and social service systems.
• Strong communication, engagement, and documentation skills.
• Ability to work independently and collaboratively as part of a team.
• Strong organizational and time management skills.
• Ability to work occasional evenings and weekends based on family needs.
• Valid driver's license.

Responsibilities

• Build supportive relationships with children, families, and caregivers.
• Collaborate with Family Support Specialists, families, and other community supports.
• Assist families in accessing community resources and services throughout the Greater New Haven area.
• Help develop strength-based, family-driven plans of care.
• Advocate for children and families across educational, healthcare, and community service systems.
• Educate, mentor, and empower families to become active participants in their own care plans.
• Transport clients when necessary and perform community-based outreach.
