Job summary

The Village for Families and Children in Hartford, CT is seeking a Parent Navigator to advocate, mentor, engage, empower and support families to take the lead in their Plan of Care. Services are in-home and/or within the community.

Responsibilities include assessing family needs, assisting with access to medical coverage, mental health and schools, and connecting families to community resources; requires a high school diploma and Spanish preferred, with a valid driver license for home visits.

Qualifications

• High school diploma or GED required.
• Bilingual Spanish preferred.

Responsibilities

• Providing assessment of family needs.
• Advocating and supporting for medical coverage, mental health, parenting education, substance abuse and schools.
• Enhancing parent's capacity to cope and manage stress related to family issues.
• Assisting families in meeting basic needs.
• Completing and maintaining all required documentation of client records and home visits.
• Maintaining collaboration with primary care providers and community services.
