The Parent Navigator will advocate, mentor, engage, empower and support families to take the lead in
their own Plan of Care and helps connect families to needed resources within their own communities.
Services are in‑home and/or within the community. Parent Navigators will help families navigate
services and assistance from community resources through putting families in touch with available
services.
