Job summary

CommuniCare in New Haven, CT is seeking a dedicated Parent Navigator to support families as they navigate local systems and services. In this community-based role, you will mentor families, advocate on their behalf, and help them lead their care plans with confidence.

Join a collaborative team to partner with families and schools and community providers. You will transport clients when needed and work evenings and weekends to meet families' needs.

Qualifications

• High school diploma with at least three years’ experience working with families.
• Knowledge of community resources and services.
• Must be available to work evenings and weekends to meet with families as needed.
• Ability to work independently and as part of a team.
• Sensitivity to diverse cultures and language competencies.
• Strong communication and documentation skills.
• Must have and maintain a valid driver’s license and be insurable at a reasonable rate under the Organization’s auto insurance plan.

Responsibilities

• Participate in child and family team meetings and ensure ongoing engagement
• Advocate for families across various systems, providers, and service settings

• Transport clients in your own vehicle when needed

In this community‑based role, you’ll mentor families, advocate on their behalf, and work with a dedicated team to help them thrive.

What You’ll Do

• Partner with families and support teams to develop strength‑based, family‑driven plans of care
• Help families access community resources across the Greater New Haven area
• Participate in child and family team meetings and ensure ongoing engagement
• Advocate for families across various systems, providers, and service settings
