Job summary

The Barren River District Health Department seeks a Family Support Worker for HANDS to review prenatal/birth records and interview parents to determine needed community support. Qualifications

• Education as listed; one year case management experience preferred.
• Master's degree can substitute for one year of experience.
• Competency in navigating community resources and referrals.

Responsibilities

• Review clinic records and conduct parent visit assessments within 30 days.
• Interview families to determine appropriate HANDS services.
• Promote outreach activities in homes and community settings.
• Develop and distribute HANDS program information materials.
• Maintain logs, prepare reports, and ensure policy compliance.
• Interpret laws and provide guidance to staff as needed.
