Job summary

Christian County Health Department is seeking a Family Support Worker to initiate and maintain support to HANDS parents, using an extensive knowledge base to enhance parenting skills and empower families. You will conduct home visits, refer to community resources, and maintain detailed case records while upholding non-judgmental and flexible approaches.

Responsibilities include monitoring family progress, coordinating referrals, and providing crisis assistance as needed.

Qualifications

• High School Diploma or GED is required.
• Three years of experience as a Family Support Worker.
• Must be 18 years of age, have telephone accessibility, reliable transportation and a valid driver’s license.
• Must be willing to work beyond normal health department hours to serve families.

Responsibilities

• Conduct initial home visit with families to establish rapport and service plan.
• Provide regularly scheduled home visits and monitor progress; document services.
• Maintain thorough, accurate, and timely documentation of each home visit.
• Collect data, assist with evaluations, and participate in supervision and mentoring.
