Meeting Street seeks a Family Support Worker for the Healthy Families America program to provide home-based, family-centered services to families with young children. The role focuses on prenatal and early childhood health education, crisis assessment, and referrals, while maintaining a caseload and reporting outcomes.

Qualifications

• Two years of providing services to and/ or engaging families with infants and toddlers in services.
• Must have experience working with families with multiple needs.
• Valid CPR and First Aid Training.
• Preference given to applicants with Growing Great Kids Curriculum or HFA core trainings.

Responsabilités

• Provide critical support to pregnant women and families with children under the age of 5.
• Conduct home visits to assess the needs of families, offer guidance, and provide family-centered, strength-based interventions to optimize parent/child relationships.
• Work with Spanish-speaking and English-speaking families to provide family-centered, strength-based interventions to optimize parent/child relationships.
• Provide curriculum-based prenatal health and early childhood health and development education to families.
• Assess additional needs or crisis situations and make referrals to other community resources.
• Maintain a caseload of families by coordinating meetings in various formats (phone, virtual, in-person).
• Document the functioning status of families and report data within program deadlines.

Formation

Bachelor's degree in Human Services, Child Development, Education or related field
