Job summary

Barren River District Health Department is seeking a HANDS Specialist to review clinic records and interview parents to assess needed community supports from birth to age three. The role involves conducting parent visit assessments and connecting families with educational and nutrition resources.

The position requires a Bachelor’s degree in a related field (or a Master’s in Human Services substitution) and/or a nursing degree with appropriate public health experience.

Qualifications

  • Bachelor’s degree in a social or behavioral science related field may be considered with 1 year of case management experience.
  • Master’s in Human Services may substitute for one year of experience.
  • Associate degree in Nursing with KY licensure and 2 years of public health experience may substitute for education.

Responsibilities

  • Conduct parent visit assessments within 30 days of receiving hospital/clinic records to determine eligibility for HANDS services.
  • Interview families to promote outreach and educate on coping techniques and accessing resources.
  • Develop and distribute informational materials that meet HANDS guidelines; maintain logs, schedules and charting.
  • Interpret laws and policies and provide guidance to staff; monitor and modify program requirements as needed.
  • Review records submitted by program staff to ensure compliance with federal and state regulations.
  • Possibly conduct ongoing home visitation as needed.

Education

Bachelor’s Degree in Social Work, Nursing, Early Childhood Education or related field
Master’s degree in Human Services (substitute for 1 year experience)
Associate degree in Nursing with Kentucky licensure (or compact state) and 2 years public health experience
