Job summary

Qualifications

• Must be 18 years of age.
• Must have telephone accessibility.
• Must have reliable transportation and valid driver's license.
• Criminal background check.
• On the job training requirements within 6 months of hire to advance: HANDS home visitor core training, HANDS wrap-around training, CPR Certification.

Responsibilities

• Conduct initial home visit following Parent Visitor assessment to establish rapport and plan services.
• Provide regular planned visits to support HANDS participants with child development and parenting skills.
• Facilitate prenatal/child health service referrals and appointments with supportive agencies.
• Document case records and logs accurately; maintain confidentiality of program information.
