Job summary

Kentucky Public Health in Morgan County seeks a HANDS home visitor (Para-Professional) to provide ongoing HANDS services to first-time and multigravida families, supporting child development, parenting skills, health services and available resources identified during visits.

Programs require on-site work with travel outside Morgan County as needed; duties include documentation, ethics, and ongoing training.

Qualifications

  • Must be 18 years of age.
  • Must have telephone accessibility.
  • Must have reliable transportation and valid Driver’s License.
  • Criminal background check.

Responsibilities

  • Conduct initial home visit following Parent Visitor assessment with first-time parents/families to establish rapport and planning for providing services.
  • Provide regular planned visits to establish a trusting relationship for HANDS participants to assist families with child development, parenting skills, health services and resources identified.
  • Facilitate families in securing prenatal/child health services, referrals, and appointments with other supportive agencies.
  • Document case records, home visit logs accurately and complete required reports.
  • Maintain confidentiality of program information obtained from home visits and exchanged with community and support providers.
  • Confer with Program Coordinator/Supervisor in cases involving potential collaboration with other service providers.
  • Complete HANDS training and training logs including HANDS core and wrap-around training.
  • Maintain required productivity per HANDS policy and report weekly on visits and schedules.
