Job summary

Urban Strategies is hiring a Family Support Specialist II in Fayetteville, NC to provide comprehensive case management and coordinate with community services under the Choice Neighborhoods Implementation grant. You will support families toward economic self-sufficiency, collaborating with social workers, relocation and employment specialists, and other professionals.

The role requires evening/weekend availability, strong communication skills, and proficiency in MS Office.

Qualifications

• Bachelor's degree in a relevant field and minimum of three years as a case manager or similar.
• Experience coordinating with social services and community partners.
• Commitment to confidentiality and ethical handling of information.
• Ability to work with diverse populations in a community setting.

Responsibilities

• Conduct intake and assessments for residents for enrollment into the case management program.
• Conduct home visits, office visits, and community site visits to collect information.
• Develop and monitor Development Plans with a multidisciplinary team.
• Navigate social service organizations and public agencies on residents' behalf.
• Identify gaps in services and coordinate service delivery for caseload.
• Maintain privacy and confidentiality of all household information.
• Enter resident data into LEARN to document progress.
• Attend community meetings with partners and other social service entities.
• Participate in program meetings and project-related sessions.
• Occasionally work nights and weekends, and travel.
• Perform administrative duties including budgets, reports, and billing.
