Job summary

Urban Strategies, Inc. in New Bern, NC is seeking a Family Support Specialist II to provide comprehensive case management, referrals, and coordination of social services under HUD funding. You will work with a team of social workers and specialists to identify strategies and support families toward economic self-sufficiency.

You will conduct intake and assessments, maintain privacy, document progress in LEARN, and attend community meetings. Evening/weekend availability and travel may be required.

Qualifications

• Bachelor’s degree in social work, Sociology, Psychology or a related field.
• Three years of work experience as a case manager or similar role with low-income households.
• Commitment to strengths-based, family-focused service planning.
• Ability to handle privileged information confidentially.
• Strong verbal and written communication skills.
• Available to attend evening and weekend events; valid driver's license.

Responsibilities

• Conduct intake and assessments for enrollment into case management.
• Perform home, office, and community visits to collect information.
• Develop, implement, monitor Development Plans with a multidisciplinary team.
• Navigate social services and public agencies to assist residents.
• Coordinate and monitor service delivery for an assigned caseload.
• Maintain privacy of all household information.
• Enter resident data into LEARN to document progress in real time.
• Attend community meetings with partners and project teams.
• Travel as required and occasional night/weekend work.
• Perform administrative duties such as reporting and billing as needed.
