Job summary

Qualifications

• Bachelor's degree required in social work, sociology, psychology or related field.
• Minimum of three years of case management or related direct practice with low-income households.
• Must protect confidentiality and handle privileged information.

Responsibilities

• Conduct intake and assessments for enrollment into the case management program.
• Perform home and office visits to collect information from residents.
• Develop and modify Development Plans with a multidisciplinary team.
• Navigate social services and public agencies to connect residents with resources.
• Maintain privacy and confidentiality of all household information.
• Enter and track resident data in the LEARN system in real time.
• Attend community meetings with partners and project staff.
• Work nights/weekends and travel as needed.
• Perform administrative tasks such as budgeting, reporting, and billing documentation.
