Job summary

Urban Strategies in Fayetteville, NC seeks a Family Support Specialist II to coordinate case management and connect residents with social services. Qualifications

• Bachelor's degree required in social work, sociology, psychology or related field.
• Experience with low-income households; able to work with diverse communities.
• Strong confidentiality and professional conduct.

Responsibilities

• Conduct intake and assessments for enrollment into the case management program.
• Conduct home, office, and community site visits to collect information.
• Develop and monitor Development Plans using a multidisciplinary approach.
• Assist residents in navigating social services and public agencies.
• Identify service gaps, advocate for residents, coordinate care for caseload.
• Enter and track resident data in LEARN to document progress.
• Attend community meetings and program-related discussions.
• Be available for occasional nights, weekends, and travel.
• Monitor budgets, prepare reports, and submit billing documentation.
