Job summary

Adopt-A-Family Of The Palm Beaches Inc is seeking a Case Manager to support at-risk families through comprehensive case management, advocacy, and service coordination. The role ensures program compliance and fosters client empowerment and stability.

The position requires a Bachelor’s in Social Work or related field and a Florida driver’s license, with at least 3 years in social services. Hybrid work, strong HMIS skills, and bilingual abilities are advantageous.

Qualifications

• Bachelor’s degree in Social Work or related field.
• Valid Florida driver’s license with insurability.
• Minimum of three (3) years’ experience in social work or human services with homeless or at-risk families.
• Knowledge of community-based organizations offering social services and specialized resources for the target population is desired.
• Experience with HMIS or client database systems.

Responsibilities

• Conduct eligibility screenings via phone, in-person visits, and documentation review.
• Develop individualized Family Action Plans with clients to promote independent living and self-sufficiency; monitor progress monthly.
• Provide ongoing case management and support through semi-monthly face-to-face meetings, including at least one monthly home visit.
• Assist families with monthly budgeting and financial planning.
• Refer clients to GED/ESOL classes, SSI/SOAR, tutoring, counseling, legal aid, and other resources.
• Advocate on behalf of clients with landlords, employers, creditors, and external agencies.
• Coordinate with community providers to deliver services and resources to families.
• Enter client data into CMIS and HMIS in a timely and accurate manner.
• Complete required reports and papers in a timely manner.
