Job summary

Adopt-A-Family of the Palm Beaches is seeking a Case Manager to support at-risk families through comprehensive case management, advocacy, and coordination of services. The role emphasizes program compliance, community partnerships, and empowering clients for long-term stability.

Responsibilities include eligibility screenings, developing Family Action Plans, coordinating home visits, budgeting assistance, and referrals to housing, education, and supportive services.

Qualifications

• Bachelor’s degree in Social Work or related field.
• Proficient in Microsoft Office Suite (Word, Excel, SharePoint, Outlook).
• Experience with HMIS or ability to learn client database systems.

Responsibilities

• Conduct eligibility screenings via phone, in-person visits, and documentation review.
• Develop individualized Family Action Plans with clients to promote independent living and self-sufficiency; monitor progress monthly.
• Provide ongoing case management and support through semi-monthly face-to-face meetings, including at least one monthly home visit.
• Assist families with monthly budgeting and financial planning.
• Refer clients to GED/ESOL classes, SSI/SOAR, tutoring, counseling, legal aid, credit counseling, parenting, life skills classes or other resources.
• Enter client data into CMIS and HMIS/Client Track systems timely and accurately.
• Complete required reports and paperwork in a timely manner and adhere to deadlines.
