SOME (So Others Might Eat) in Washington, D.C. is seeking a Family Services Case Manager - Adult Services Coordinator to provide case management and supportive services for residents in family housing.

You will develop and coordinate life-skills programming and professional development to empower adults and families. Responsibilities include goal planning, resource navigation, program activity development, and outreach to partner organizations.

Qualifications

• High School Diploma/GED required; 5-8 years' experience in social services and case management experience.
• Graduate level licensure required for Master's degree in Social Work or Counseling related program (LGSW, LGPC, etc.).
• CPR/First-Aid, CPR for Infants.
• Background checks and references required; two-tier interview process.

Responsibilities

• Provide case management services utilizing strength-based perspective and motivational enhancement theory.
• Collaborate with families to develop goals, plans and monitor progress.
• Identify, educate and teach residents to access community resources.
• Implement clinical interventions as needed.
• Meet with families/individuals for case management at regular intervals.
• Develop and coordinate activities for adults on life skills and professional development.
• Promote, market and ensure attendance for program activities and initiatives.
