Job summary

Community Partnership for Families is seeking a non-exempt Case Manager to coordinate services within a neighborhood-based family resource center. You will join a multidisciplinary team and manage referrals, assessments, and service provision through to aftercare.

The role requires strong communication, cultural sensitivity, and ability to engage families, with some evening/weekend work as needed. Minimum four years in community work or an 8-year nonprofit track apply.

Qualifications

• Experience providing comprehensive case management services.
• Ability to communicate with diverse audiences and cultures.

Responsibilities

• Performs case management and administrative functions as part of an integrated service team.
• Provide case management to systemic impacted individuals and families.
• Maintains accurate electronic case files in a timely manner.
