Job summary

Easterseals Florida is seeking a compassionate Family Service Coordinator to support families of young children through the Space Coast Early Steps program. The FSC will coordinate evaluations, services, and transitions while serving as a central contact for families from start to end of their Early Steps journey.

You will work with a multidisciplinary team to ensure timely evaluations, services, procedural safeguards, and coordinated care, advocating for services that promote child development.

Qualifications

• Bachelor's degree in a related field.
• At least 1 year of professional experience with families of young children.
• Ability to secure Medicaid Provider Number.
• Knowledge of community resources, developmental topics, and privacy laws.
• Strong documentation, time management, and organizational skills.

Responsibilities

• Provide ongoing service coordination for children eligible for Early Steps.
• Participate in multidisciplinary evaluations and IFSP meetings.
• Support eligibility determinations and transitions from IDEA Part C.
• Coordinate IFSPs and communicate recommendations with providers.
