Job summary

Easterseals Florida, Inc is seeking a compassionate and dedicated Bilingual (Spanish/English) Family Service Coordinator to support families of young children eligible for IDEA, Part C services through our North Beaches Early Steps program. You will serve as the central contact from start to end, coordinating evaluations, services, and transitions with a multidisciplinary team.

You will travel to family homes, develop IFSPs with families and team members, and advocate for services and resources.

Qualifications

• Bachelor's degree from an accredited university in psychology, social work, early childhood, child development, special education, health education, sociology, or a related field.
• One year of professional experience working with young children and families in teaching, case management, counseling, or serving individuals with special needs or developmental delays.
• Ability to secure and maintain a Medicaid Provider Number.
• Knowledge of community resources, child development, disabilities, and confidentiality/privacy laws.
• Strong documentation, time-management, and organizational skills.
• Ability to travel to family homes and adjust schedule to meet family needs.
• Ability to provide crisis management support when necessary.
• Demonstrates integrity, tact, strong judgment, flexibility, and a commitment to self-development and quality practices.
• Commitment to teamwork and to supporting families with empathy and professionalism.
• Ability to meet our approved driver qualifications in order to drive your personal vehicle, as required, for program-related needs (minimum age of 21 and good driving history).

Responsibilities

• Provide ongoing service coordination for children eligible for Early Steps as the assigned service coordinator.
• Participate in multidisciplinary evaluations, IFSP meetings, and transition planning activities.
• Support eligibility determinations, monitor interventions, and assist families through transitions from IDEA Part C.
• Promote and strengthen the IFSP team's ability to identify, access, coordinate, and evaluate services and resources.
• Interview families to gather information on routines, strengths, needs, support systems, and developmental concerns.
• Develop timely and comprehensive Individualized Family Support Plans (IFSPs) in collaboration with families and team members.
• Communicate IFSP recommendations with providers, coordinate updates every 6 months, and ensure follow-up for developmental evaluations.
• Coordinate transitions to Part B for families choosing school-based services, including required notifications and planning.
• Assist families not pursuing Part B with developing an appropriate post-Part C plan.
• Maintain accurate case notes, file documentation, and billing practices for CMS Targeted Case Management.
• Travel to family homes orPreferred locations to conduct interviews, gather information, and complete coordination tasks.
