Job summary

Easterseals Florida is seeking a compassionate Family Service Coordinator to support families of young children eligible for IDEA, Part C through the Space Coast Early Steps program. The FSC will be the central contact, coordinating evaluations, services, and navigation of resources to promote child development.

As part of a dedicated team, you will travel to families’ homes, develop timely IFSPs, and advocate for services while ensuring accurate documentation and billing.

Qualifications

• Bachelor’s degree in psychology, social work, early childhood, child development, special education, health education, sociology or related field.
• One year of professional experience with young children and families in teaching, case management, counseling, or related settings.
• Ability to obtain Medicaid Provider Number.
• Knowledge of community resources, child development, disabilities and privacy laws.
• Strong documentation, time-management and organizational skills.
• Willingness to travel to family homes as needed.
• Commitment to teamwork and empathetic, professional support for families.

Responsibilities

• Provide ongoing service coordination for children eligible for Early Steps.
• Participate in multidisciplinary evaluations, IFSP meetings and transition planning.
• Support eligibility determinations, monitor interventions, and assist transitions from IDEA Part C.
• Promote the IFSP team’s ability to identify, access and evaluate services and resources.
• Interview families to gather information on routines, strengths and needs.
• Develop IFSPs in collaboration with families and team members.
• Communicate IFSP recommendations with providers and coordinate updates every 6 months.
• Coordinate transitions to Part B for school-based services.
