Job summary

Easterseals Florida is seeking a compassionate Bilingual (Spanish/English) Family Service Coordinator to support families of young children eligible for IDEA Part C services through the North Beaches Early Steps program. The FSC will coordinate evaluations, services, safeguards, and coordinated care with a comprehensive team.

You will work to empower families, helping them understand their child’s strengths and needs, and advocate for services that promote optimal development.

Qualifications

• Bilingual in Spanish and English required.
• Bachelor's degree in a relevant field (e.g., psychology, social work, early childhood).
• One year of professional experience with young children and families.
• Ability to secure and maintain a Medicaid Provider Number.

Responsibilities

• Provide ongoing service coordination for children eligible for Early Steps as the assigned service coordinator.
• Participate in multidisciplinary evaluations, IFSP meetings, and transition planning activities.
• Support eligibility determinations, monitor interventions, and assist families through transitions from IDEA Part C.
• Promote and strengthen the IFSP team's ability to identify, access, coordinate, and evaluate services and resources.
• Interview families to gather information on routines, strengths, needs, support systems, and developmental concerns.
• Develop timely and comprehensive IFSPs in collaboration with families and team members.
• Communicate IFSP recommendations with providers, coordinate updates every 6 months, and ensure follow-up for developmental evaluations.
• Coordinate transitions to Part B for families choosing school-based services, including required notifications and planning.
• Assist families not pursuing Part B with developing an appropriate post-Part C plan.
• Maintain accurate case notes, file documentation, and billing practices for CMS Targeted Case Management.
• Travel to family homes or preferred locations to conduct interviews, gather information, and complete coordination tasks.
