Job summary

Easterseals Florida is seeking a compassionate bilingual (Spanish/English) Family Service Coordinator to support families of young children eligible for IDEA Part C through the North Beaches Early Steps program. As the central point of contact, the FSC will help families understand their child’s strengths, navigate services, and coordinate evaluations, plans, and transitions with a multidisciplinary team.

This full-time role (40 hours weekly, 9:00–5:00) offers a salary of $44,000 per year plus

Qualifications

• Bilingual (Spanish/English) required.
• Bachelor’s degree in a related field.
• One year of professional experience with young children and families.
• Ability to secure Medicaid Provider Number.
• Knowledge of community resources, child development, disabilities, and confidentiality/privacy laws.
• Strong documentation, time-management, and organizational skills.
• Ability to travel to family homes and adjust schedule to meet family needs.
• Demonstrates integrity, tact, strong judgment, flexibility, and a commitment to self-development and quality practices.

Responsibilities

• Provide ongoing service coordination for Early Steps children.
• Participate in multidisciplinary evaluations, IFSP meetings, and transition planning activities.
• Support eligibility determinations, monitor interventions, and assist families through transitions from IDEA Part C.
• Promote and strengthen the IFSP team’s ability to identify, access, coordinate, and evaluate services and resources.
• Interview families to gather information on routines, strengths, needs, support systems, and developmental concerns.
• Develop timely and comprehensive IFSPs in collaboration with families and team members.
• Communicate IFSP recommendations with providers, coordinate updates every 6 months, and ensure follow-up for developmental evaluations.
• Coordinate transitions to Part B for families choosing school-based services, including required notifications and planning.
• Assist families not pursuing Part B with developing an appropriate post-Part C plan.
• Maintain accurate case notes, file documentation, and billing practices for CMS Targeted Case Management.
• Travel to family homes or preferred locations to conduct interviews, gather information, and complete coordination tasks.
